We are a small agency located in Albany, New York. We have a client seeking an RN-Coordinator of Care.

Their range of health services include home care, hospice and palliative care for adults and
children, rehabilitation and nursing care and the research-based institution for innovation
in palliative care. They also offer health plans for Medicare and dual-eligible individuals.
Their ability to serve at-risk members of the community is supported by the efforts of their
non-for-profit foundation, which raises funds to help support their quality programs and
services not reimbursed by insurance or government programs. In addition, the generosity
of grateful families, private and corporate donors and grants, as well as their own
employees, also support their specialized services.
Responsibilities:
In this role, you will assume the ongoing, primary responsibility to coordinate, implement
and continually evaluate the home care needs of your patients. You will also review patient
insurance information; coordinate activities involved in each individual care plan and make
sure that pertinent findings are shared with family and professional caregivers. Your
excellent communication, documentation and time management capabilities will be
essential to your success. And all along the way, you will experience the respect for your
opinions and the high level of autonomy you need to do the job right
Qualifications:
• Graduate from an accredited School of Nursing; BSN preferred
• Minimum of 1 year acute, Med-Surg Nursing experience – required
Shift: Monday – Friday, 9:00 am – 5:00 pm
Salary: $100,000 - $119,000
